Book Report Forms

Next year we are venturing into what I consider to be new and unchartered territory for our family in our homeschooling journey. We will have a 4th and 2nd grader (yikes when did the kids grow up so quickly!). I always like to have the basic forms on hand to use throughout a school year so that I am not spending limitless time searching on the computer for a form to use with a lesson right before I need it.

Since we are still on both ends of a typical elementary school setting I created both an Upper Elementary Book Report Form as well as a Lower Elementary Book Report Form. The Upper Elementary Book Report Form was created for grades 3-5. The Lower Elementary Book Report Form was created for grades 1 -3. My hope in creating these forms is to present my children with a way to share what they have learned in the story they are reading, provide writing practice, and most importantly keep an eye on their reading comprehension to make sure that they are not skipping over important details in the story.
